b) Request for approval of minutes for the meeting held on May 15, 2026, at 8:00 a.m.
c) Request for the approval of check register dated May 1, 2026, to May 31, 2026, including voided checks 140144, in the amount of $1,601,936.60.
d) Credit Card Purchases for April 2026.
e) Request to approve the purchase orders:
1. PO#4959-Utah Transit Authority for Bus passes for WIN, GSA, and ATR/PATR/SUD RSS Clients. July 2026 - June 2027, in the amount of $20,000.00.
2. PO#4961- Insight Direct for Microsoft SQL Server Standard Core Edition - software assurance, in the amount of $31,673.12.
3. PO#4962- APSI for a weeklong training on media strategies, evaluation, implementation, and selection of evidence-based Media efforts, in the amount of $22,684.00.

a) Request to ratify the Agreement as presented.
1. Agreement between Weber Human Services and Problems Anonymous Action Group, Inc.
This Agreement is made July 1, 2025, by and between Weber Human Services and Problems Anonymous Action Group, Inc. Whereas, PAAG shall provide housing for twenty persistently mentally ill clients who are unable to live in the mainstream of the community. PAAG will provide lunch meals for up to forty individuals who are receiving day treatment services at WHS or socialization opportunities at the PAAG drop-in center. WHS will pay PAAG monthly, $8,000.00 for housing of 20 SPMI clients and $2,686.00 for lunch meals for Day Treatment and Drop-In Center Clients.
b) Request to approve the Agreement as presented.
1. Agreement for Legal Services
This Agreement is made and entered into by and between Weber Human Services (WHS) and Flint Legal, PLLC DBA FW Law (Attorney). The Attorney shall act as general legal counsel to WHS and shall perform specific duties and responsibilities. WHS shall pay Attorney $4,200.00 per month during the term of this Agreement. Said amount shall be billed to WHS quarterly.
c) Request to ratify the FY2027 tentative budget as presented.
Due to the lack of a quorum, no vote was held on the tentative budget during the meeting; however, the tentative budget was subsequently approved by email vote.
d) Request to approve the FY2027 budget as presented.
