NOTICE OF PUBLIC HEARING
NOTICE IS HEREBY GIVEN that a designated hearing officer of the State of Utah will hold a
public hearing at 10:00 a.m. local time on Wednesday, April 15, 2026, in the office of the State
Treasurer of the State of Utah, 350 North State Street, Suite 180, Salt Lake City, Utah. In addition,
the public hearing will be simultaneously accessible by residents, taxpayers and other interested
persons telephonically on the date and at the time listed above by dialing into the toll-free telephone
conference line number: (877) 853-5247, and entering access code: 3639026303#. The public
hearing will be held with respect to a plan by the Colorado Health Facilities Authority to issue its
Revenue Bonds (Intermountain Health) in one or more series over the longest period permitted by
law and in an aggregate principal amount not to exceed $3,500,000,000 with respect to facilities
located in the State of Utah. The proceeds of the bonds issued under such plan (the 'Bonds') will
be used to (a) finance, refinance, or reimburse the costs of acquiring, constructing, renovating and
equipping certain health care facilities (the 'Health Care Facilities') at the locations listed below
(collectively, the 'Projects'), (b) refund certain revenue bonds and/or taxable indebtedness
previously issued to finance or refinance the costs of acquiring, constructing, renovating and
equipping certain of the Health Care Facilities (collectively, the 'Prior Debt'), (c) provide one or
more debt service reserve funds for the benefit of all or a portion of the Bonds, if deemed necessary
or desirable, (d) pay a portion of the interest to accrue on the Bonds, if deemed necessary or
desirable, (e) pay certain working capital expenditures, if deemed necessary or desirable, and (f)
pay certain costs of issuance of the Bonds, including the costs of any credit or liquidity
enhancement thereof, if deemed necessary or desirable.
The Bonds will be qualified 501(c)(3) bonds (as defined in Section 145 of the Internal
Revenue Code of 1986, as amended), and the proceeds of the Bonds were or will be used to
finance, refinance or reimburse the costs of acquiring, constructing, improving or renovating the
Health Care Facilities (including related land costs) listed below and the costs of acquiring and
installing equipment (including, but not limited to, medical equipment, computer equipment, office
equipment and general building equipment and fixtures) used or to be used at the Health Care
Facilities listed below. A portion of the Health Care Facilities listed below are also being
refinanced with the proceeds of the Bonds through the refunding of the Prior Debt. The initial
owner or principal user of the Health Care Facilities more specifically described below is IHC
Health Services, Inc., a Utah nonprofit corporation. A general functional description of the type
and use of each Health Care Facility, the location of each such Health Care Facility, and the
estimated maximum aggregate principal amount of Bonds to be issued with respect to new money
Projects to be undertaken at each such Health Care Facility are listed below:
1. LDS Hospital, a new 218-bed general acute-care replacement hospital and the related
land, parking garages, office buildings, wellness centers, hospice facilities, medical education
facilities, clinics, warehouses, and other buildings located on the new LDS Hospital campus, with
a primary mailing address of 754 South State Street in Salt Lake City, Utah - $843,000,000;
2. Intermountain Medical Center, a 510-bed general acute-care hospital and the related
land, parking garages, office buildings, wellness centers, hospice facilities, medical education
facilities, clinics, warehouses, and other buildings, all located on the Intermountain Medical Center

campus. This campus is located directly west of State Street, directly east of Cottonwood Street,
and bordered generally by Vine Street and 5300 S, with a primary mailing address of 5121
Cottonwood Street in Murray, Utah - $607,000,000;
3. Primary Children's Hospital, a 264-bed children's hospital and the related land,
parking garages, office buildings, medical education facilities, clinics, warehouses, and other
buildings, all located on the Primary Children's Hospital Salt Lake City campus, with a primary
mailing address of 100 Mario Capecchi Drive in Salt Lake City, Utah - $271,000,000;
4. Utah Valley Hospital, a 398-bed general acute-care hospital and the related land,
parking garages, office buildings, wellness centers, hospice facilities, medical education facilities,
clinics, warehouses, and other buildings, all located on the Utah Valley Hospital Campus located
directly east of 500 W, bordered generally by West Cougar Boulevard on the north, 800 North on
the south, and 300 West on the east, with a primary mailing address of 1034 North 500 West in
Provo, Utah - $134,000,000;
5. McKay-Dee Hospital, a 319-bed general acute-care hospital, outpatient facility, and
the related land, parking garages, office buildings, wellness centers, hospice facilities, medical
education facilities, clinics, warehouses, and other buildings located on the McKay-Dee Hospital
main campus, with a primary address of 4401 Harrison Boulevard, and on the McKay-Dee
Hospital north campus, with a primary address of 3903 Harrison Boulevard in Ogden, Utah -
$159,000,000;
6. St. George Regional Hospital, a 334-bed general acute-care hospital and the related
land, parking garages, office buildings, wellness centers, hospice facilities, medical education
facilities, clinics, warehouses, and other buildings, all located on the St. George Regional Hospital
campus directly south of Medical Center Drive, directly north of Foremaster Drive, and bordered
generally by River Road, with a primary mailing address of 1380 East Medical Center Drive in St.
George, Utah - $135,000,000;
7. Desert Color Campus, a new approximately 19,000 square foot health care facility
and the related land, parking garages, office buildings, wellness centers, hospice facilities, medical
education facilities, clinics, warehouses, and other buildings, including but not limited to the
construction, renovation, and equipping of a 15,400 square foot clinic to be located on a parcel of
land consisting of approximately 38 acres directly south of Desert Color Parkway and Lagoon
Parkway in St. George, Utah - $68,000,000;
8. Ephraim Hospital, a new 55,000 square foot acute-care hospital and 20,000 square
foot clinic and the related land, parking garages, office buildings, wellness centers, hospice
facilities, medical education facilities, warehouses, and other buildings to be located on
approximately 38 acres on the northwest corner of Main Street and Kiesel Lane in Ephraim, Utah
- $104,000,000;
9. Primary Children's Hospital - Lehi, a 66-bed children's hospital and the related land,
parking garages, office buildings, medical education facilities, clinics, warehouses, and other

buildings all located on the Primary Children's Hospital Lehi campus, with a primary mailing
address of 2250 North Miller Campus Drive in Lehi, Utah - $25,000,000;
10. Logan Regional Hospital, a 146-bed general acute-care hospital and the related land,
parking garages, office buildings, wellness centers, hospice facilities, medical education facilities,
clinics, warehouses, and other buildings located on the Logan Regional Hospital campus, bordered
generally by 1400 North on the north, 1200 North on the south and 600 East on the east, with a
primary mailing address of 500 East 1400 North in Logan, Utah - $38,000,000;
11. Intermountain Sports Medicine and Performance Center, a new 60,000 square foot
clinic and the related ambulatory surgery centers, parking garages, office buildings, wellness
centers, medical education facilities, warehouses, and other buildings, located to the southwest of
10400 South and Mall Ring Road in Sandy, Utah - $65,000,000;
12. Park City Hospital, a 37-bed general acute-care hospital and the related land, parking
garages, office buildings, wellness centers, hospice facilities, medical education facilities, clinics,
warehouses, and other buildings located on the Park City Hospital campus, with a primary mailing
address of 900 Round Valley Drive in Park City, Utah - $72,000,000;
13. Spanish Fork Hospital, a 33-bed general acute-care hospital and the related land,
parking garages, office buildings, wellness centers, hospice facilities, medical education facilities,
clinics, warehouses, and other buildings located at 765 East Market Place Drive in Spanish Fork,
Utah - $49,000,000;
14. Cedar City Hospital, a 48-bed general acute-care hospital and the related land,
parking garages, office buildings, wellness centers, hospice facilities, medical education facilities,
clinics, warehouses, and other buildings located on the Cedar City Hospital campus, with a primary
mailing address of 1303 North MainH Street, including 1333 North Main Street, in Cedar City,
Utah - $68,000,000;
15. Alta View Hospital a 51-bed general acute-care hospital and the related land, parking
garages, office buildings, wellness centers, hospice facilities, medical education facilities, clinics,
warehouses, and other buildings located on the Alta View Hospital campus, with a primary mailing
address of 9660 South 1300 East in Sandy, Utah - $30,000,000;
16. Saratoga Springs Emergency Department and Outpatient Facility, a 22,000 square
foot free-standing emergency department and outpatient health care facility and the related land,
parking garages, office buildings, wellness centers, hospice facilities, medical education facilities,
ambulatory surgery centers, clinics, warehouses, and other buildings, including but not limited to
the construction, renovation and equipping of a 46,000 square foot clinic, located on the Saratoga
Springs campus on approximately 41 acres of land on the northeast corner of Pioneer Crossing and
Medical Drive, with a primary mailing address of 392 West Medical Drive in Saratoga Springs,
Utah - $67,000,000;
17. The Orthopedic Specialty Group, a specialty medical center, outpatient facilities and
the related land, parking garages, office buildings, wellness centers, hospice facilities, medical

Group campus directly west of Fashion Boulevard (300 E), bordered generally by McMillan Lane

18. American Fork Hospital, a 70-bed general acute-care hospital and the related land,

directly east of 1100 East and bordered by 90 North and 200 North, with a primary mailing address

27. Sevier Valley Hospital, a 29-bed general acute-care hospital and the related land,

and other buildings, to be located on approximately 5 acres on the northeast corner of Washington
Parkway and Grapevine Crossing Road in Washington, Utah - $19,000,000;
32. Erda Clinic, a 14,000 square foot clinic and the related land, parking garages, office
buildings, wellness centers, hospice facilities, medical education facilities, clinics, warehouses,
and other buildings located on the southeast corner of Highway 36 and Erda Way in Erda, Utah -
$11,000,000;
33. Primary Children's Wasatch Canyons, a 102,000 square foot children's behavioral
health facility and the related land, parking garages, office buildings, medical education facilities,
clinics, warehouses, and other buildings to be located on the Primary Children's Wasatch Canyons
campus, with a primary mailing address of 5770 South 1500 West in Taylorsville, Utah;
34. Bountiful Clinic, a 118,000 square foot clinic and the related land, parking garages,
office buildings, wellness centers, hospice facilities, medical education facilities, clinics,
warehouses, and other buildings located at 390 North Main Street in Bountiful, Utah;

36. Washington Fields Clinic, a 18,000 square foot clinic and the related land, parking

warehouses, and other buildings located at 2947 East 1450 South in St. George, Utah;
37. Lehi Clinic, an 18,000 square foot clinic and the related land, parking garages, office
buildings, wellness centers, hospice facilities, medical education facilities, clinics, warehouses,
and other buildings located at 3249 North 1200 West in Lehi, Utah;
38. Layton Surgical Center, a surgical center and the related land, parking garages, office
buildings, wellness centers, hospice facilities, medical education facilities, clinics, warehouses,
and other buildings located at 201 West Layton Parkway in Layton, Utah;
39. West Jordan Clinic, a 9,000 square foot clinic and the related land, parking garages,

warehouses, and other buildings, located at 2655 West 9000 South in West Jordan, Utah;

The public hearing is required by Section 147(f) of the Internal Revenue Code of 1986, as
