1.	Consent Calendar: 

a)	Request for approval of minutes for meeting held on October 21, 2016.

b)	Request for approval of check register dated October 1, 2016 through October 31, 2016 including voided checks 97783, 101196, 101580 and 101718; covering checks numbered 101838 through 102285 in the amount of $891,704.35.

c)	Request for approval of check register dated November 1, 2016 through November 30, 2016 including voided checks 97330, 99670, 101771, 101815, 101991 and 102249; covering checks numbered 102286 through 102738 in the amount of $1,853,165.60.

d)	Request to ratify purchase orders.
1.	PO # 4223 to eCenter Research for Adult and Youth Substance abuse team licenses in the amount of $11,040.00.
2.	PO # 4225 to Superior Computers-Ogden for five (5) Intel Core i5-6600 Quad Core Processors in the amount of $4,650.00.
3.	PO # 4226 to Allied Electric Signs for signage in the amount of $17,265.00. 

e)	Request to approve purchase orders.
1.	PO # 4227 to Converge One for switches, cables, and maintenance in the amount of $9,833.64.
2.	PO # 4228 to MARSH USA for directors, officers, general, professional, and professional liability excess coverage insurance renewal 2017 in the amount of $228,730.00.
3.	PO # 4229 to Utah Counties Indemnity Pool for workers compensation, multiline pool member contribution coverage 2017 in the amount of $126,573.11.

a)	Request to ratify contracts as presented

1.	PFS Amendment #3 Contract No. 160382
This contract is between the State of Utah, Department of Human Services, Division of Substance Abuse and Mental Health (DHS/DSAMH; and Weber Human Services (Contractor). The purpose of this amendment: to 1) add $57,019.00 in funding for FFY17; 2) replace the rate table; and 3) add the FFY17 DHS Budget Statement forms. Contract Period: 07/01/2015 (original starting date), 09/30/2020 (current ending date). Amendment Amount: $211,849.00 (current contract amount), $57,019.00 (amendment amount) for funding period 10/1/16 - 9/30/17, $268,868.00 new contract amount. 

2.	Utah Transit Authority Contract No. 16-1883JH
This agreement is between Davis County, through the Davis County Senior Services, Weber County Aging Services, Roads to Independence, a Utah nonprofit corporation (Collectively Contractors) and Utah Transit Authority, a public transit district organized under the laws of the State of Utah (UTA).  The parties desire to secure and utilize grant funds for the transportation needs of elderly persons and persons with disabilities on the State of Utah. 

3.	Contract for Day Care Services
This contract is entered into by Little Cubs Preschool and Daycare (Provider) and Weber Human Services (Client) on this date, October 1, 2016. The terms of this contract will be valid for 6 months and will end on June 30, 2017. Either party may terminate this contract with at least 30 days written notice to the other party. Scope and Manner of Services: Guaranteed slots for day care services for 3 children. The guaranteed slots will be set for children ages 0-2 years. Ages of children may vary but the Provider will ensure that 3 slots for this age group will always be available. Transportation to and from day care for all children receiving day care services will be provided from Client. The Client shall pay the Provider for services rendered according to the Payment Schedule. 3 Guaranteed Slots: $630.00 per month for a total of $1890.00 per month. 

4.	Golden Hours Senior Center  Interlocal Agreement - ROLL CALL VOTE NEEDED
Interlocal Cooperation Agreement is between Weber Human Services and Ogden City for services to the Golden Hours Senior Center. WHS desires to cooperatively work with the City to provide services to the senior citizens of Ogden City, Weber County and Morgan County, which services shall include nutritional, educational, transportation, health promotion and socialization programs and activities for old individuals. This agreement shall be for a period of thirty six (36) months commencing on 1 July 2016, and ending on 30 June 2019. This agreement may be terminated at any time by either Party without cause upon three (3) month's prior written notice to the other party.   

5.	Prepaid Mental Health Plan- Weber Human Services  State Contract No. 172700414
This contract is between the Utah Department of Health (Department) and Weber Human Services (Contractor). The general purpose of this contract is to provide inpatient mental health and outpatient mental health and substance use disorder services. The service period of this contract is 11/01/2016 through 06/30/2021, unless terminated or extended by agreement in accordance with the terms and conditions of this contract. The Department agrees to pay $122,200,000.00 in accordance with the provisions of this contract. This contract is funded with 70% federal funds, 30% state funds, and 0% other funds. 

b)	Request to approve contracts as presented

1.	IHC Health Service, Inc.
This Amendment is effective on the first day of January, 2017, and is made to amend the Medicaid Subcontract Agreement dated April 1, 2013, by and between IHC Health Services, Inc. (Contractor), a Utah nonprofit corporation and Weber Human Services (WHS). The parties agree to amend the Agreement as follows: Section III., 'Contract Period,' and Section 16., 'Direct Payment'. 

2.	Community Level PDO- Weber Human Services  State Contract No. 172700502
This contract is between the Utah Department of Health (Department) and Weber Human Services (Contractor). The general purpose of this contract is to enhance and empower community-level prevention and intervention that decreases opioid abuse, emergency department visits, overdoses, and other indicators of prescription drug overdose burden. The service period of this contract is 08/31/2016 through 08/31/2019, unless terminated or extended by agreement in accordance with the terms and conditions of this contract. The Department agrees to pay $239,742.00 in accordance with the provisions of this contract. This contract is funded with 100% federal funds, 0% state funds, and 0% other funds. 

c)	WHS Image: Signage update and new agency logo discussion. (Board approved authorizing E.D. to move forward on signage due to no November meeting) 

d)	Discussion on accepting new logo. Keep existing logo, adopt newly proposed, continue looking? 

e)	Bob Hunters board term: January 1,2015 - December 31, 2016
