Agenda Board of Directors Meeting Weber Human Services 3rd floor, Board of Directors Room Commencing at 8:00 A.M. April 18, 2025. 1. Public Comments: 2. Consent Calendar: a) Welcome b) Request for approval of minutes for meeting held on March 21, 2025, at 8:00 a.m. c) Request for the approval of check register dated March 1, 2025, to March 31, 2025, including voided checks 129179, 129209, 136462, 136488, 136538 and 136610 in the amount of $1,583,991.63. d) Credit Card Purchases for February 2025. e) Request to ratify the purchase orders: 1. PO#4893-Midwest Commercial Interiors for Kitchen Remodel Furniture, in the amount of $32,533.82. 2. PO#4894-Alt Architecture for Case Management, Additional Structural Design and PHBH Clinic fees, in the amount of $10,366.00. f) Request to approve the purchase orders: 1. PO#4895-Ogden Raptors for 2 Baseball games including, video on the jumbotron, food vouchers, 1000 tickets, in the amount of $11,000.00 2. PO#4896-Insight for 20 Lenovo ThinkBook 16 Generation 6 Irl Touchscreens and 3-year extended warranties, in the amount of $20,589.20. 3. Action Items a) Request to approve the WHS Travel Policy as presented. (1st and 2nd Reading). b) Request to approve the WHS Mission Statement as presented. (1st and 2nd Reading). c) Request to approve the FY2026 Aging Area Plan. 4.
