This Cooperative Agreement seeks to prevent or delay onset of type 2 diabetes by decreasing risk among adults with prediabetes and improve self-care practices, quality of care, and early detection of complications among people with diabetes. All work supported under this Cooperative Agreement will focus on reducing health disparities and achieving health equity for priority populations, defined as those who have systematically experienced greater obstacles to health. DHHS will work closely with LHDs and other contracted partners to implement a variety of strategies to decrease risk for type 2 diabetes among adults at high risk; improve selfcare practices, quality of care, and early detection of complications among priority populations with diabetes.
Improving Student Health and Academic Achievement through Nutrition, Physical Activity and the Management of Chronic Conditions in Schools

The purpose of this 5-year cooperative agreement is to protect and improve the health and wellbeing of school-age children and adolescents in underserved and disproportionately affected communities. Recipients will use the Whole School, Whole Community, Whole Child (WSCC) model to: establish key partnerships to support school health programs; promote dissemination and implementation of CDC school health guidelines, tools, and resources through Professional Development (PD) and Technical Assistance (TA); and use action planning to implement physical activity, school nutrition, and school health policies, practices, programs, and services within state and local education agencies (LEAs), schools, and out-of-school time (OST) settings.

Support the following cancer prevention and control goals: eliminate preventable cancers, ensure all people get the right screening at the right time for the best outcome, and support cancer survivors in a manner that allows them to live longer, healthier lives. This program takes a comprehensive, coordinated approach to achieving these goals and advance health equity. It supports breast and cervical cancer screening services, cancer control plan implementation by coalitions, and surveillance programs to monitor and report cancer burden.
