1. Prevention Prepared Communities Grant Contract between Bonneville Communities That Care and Uintah City.
This Contract is between Bonneville Communities That Care and Uintah City. The purpose of this contract is the provision of evidence-based substance use disorder prevention services in the Bonneville Communities That Care coalition communities using Community Centered Evidence Based Prevention through Uintah City. Responsibility of Bonneville Communities That Care Coalition is to provide $9,000 between April 1, 2023-March 31, 2024, to meet the Prevention-Prepared Community Grant requirements.
2. Prevention Prepared Communities Grant Contract between Bonneville Communities That Care and Riverdale City.
This Contract is between Bonneville Communities That Care and Riverdale City. The purpose of this contract is the provision of evidence-based substance use disorder prevention services in the Bonneville Communities That Care coalition communities using Community Centered Evidence Based Prevention through Riverdale City. Responsibility of Bonneville Communities That Care Coalition is to provide $9,000 between April 1, 2023-March 31, 2024, to meet the Prevention-Prepared Community Grant requirements.
3. Prevention Prepared Communities Grant Contract between Bonneville Communities That Care and South Ogden City.
This Contract is between Bonneville Communities That Care and South Ogden City. The purpose of this contract is the provision of evidence-based substance use disorder prevention services in the Bonneville Communities That Care coalition communities using Community Centered Evidence Based Prevention through South Ogden City. Responsibility of Bonneville Communities That Care Coalition is to provide $9,000 between April 1, 2023-March 31, 2024, to meet the Prevention-Prepared Community Grant requirements.
4. Prevention Prepared Communities Grant Contract between Bonneville Communities That Care and Bonneville High School.
This Contract is between Bonneville Communities That Care and South Ogden City. The purpose of this contract is the provision of evidence-based substance use disorder prevention services in the Bonneville Communities That Care coalition communities using Community Centered Evidence Based Prevention through Bonneville High School. Responsibility of Bonneville Communities That Care Coalition is to provide $9,000 between April 1, 2023-March 31, 2024, to meet the Prevention-Prepared Community Grant requirements.
b) Request to approve the Agreement as presented.
1. Agreement between Weber Human Services and Latitude for the provision of Home and Community Based Services.
This Agreement made by and between Weber Human Services (WHS) and Latitude (Provider), whereas the parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 or over) of Weber and Morgan Counties. This agreement will be for a period of 16 months commencing on March 1, 2024, and terminating on June 30, 2025, unless terminated sooner by either party. The rates for this agreement will be as follows: Emergency Response System (one month) $37.00 for monitoring or 6 chosen contacts (includes auto-alert, GPS, Geofencing, and mobile capabilities. 1 time payment (ERS) $447.00 for Monitoring or 6 chosen contacts (includes auto-alert, GPS, Geofencing, and mobile capabilities. 1 time payment (ERS) $45.00 starting 2nd year (cover sim card) pay 1 time each year its activated.
