2. Action Items
a) Request to ratify the contracts as presented.
1. Weber SFY2024 AAA Amendment #9 Memo
The purpose of this contract amendment memo is to inform of any increase to the Pass-Through Contract A03148 base budget amount for State Fiscal Year 2024. The Weber County Area Agency on Aging 'AAA' contract will increase by $2,242,528.00 for SFY 2024. The required local cash match is $257,271.76 for SFY2024. The new contract total is $9,151,625.00 for contract A01348. This is made effective of July 1, 2023.
2. Weber FSY2024 Waiver Amendment #5 contract A03198
The purpose of this contract amendment memo is to inform you of your Medicaid Aging Waiver budget for State Fiscal Year 2024. The funding amounts are listed as follows: Service Dollars $511,500.00, Administration Dollars base amount $56,300.00, Administration Dollard discretionary amount $9,900.00, Personal Assistant Fees $400.00, Beginning Fiscal Year Slot Amount $50.00. This is made effective July 1, 2023.
3. Agreement between Episcopal Management Corporation of Ogden, Inc. and Weber Human Services
This Agreement is made effective July 1, 2023, by and between Episcopal Management Corporation of Ogden, Inc. (Danville Development) (hereinafter referred to as 'owner') and Weber Human Services (hereinafter referred to as WHS). The partis hereto are mutually desirous of affiliating to provide limited-service coordination services for the residents of Union Gardens. Owner agrees to pay WHS for services as follows: Service Coordination rendered on an hourly basis, at the rate of $32.77 per hour with a maximum of 30 per week of 1560 hours annually, which will include benefits when necessary. Travel Reimbursement at $.0655 per mile for the service coordinator to travel to local trainings and meetings in any designated location. This Agreement shall be for a period of 12 months commencing on July 1, 2023 and terminating on the 30th day of June 2024.
b) Request to approve the Contracts as presented.
1. Amendment to the Medicaid Subcontract Agreement
This Agreement if effective July 1, 2023, by and between IHC Health Services, Inc. dba Intermountain Medical Group (Contractor) and Weber Human Services (WHS), whereas Section III, 'Contract Period' of the Agreement is deleted in its entirety and replaced with the following Section III, 'Contract Period' to extend the term of the Agreement. Contract Period: This contract if effective April 1, 2013 and shall continue in effect until the earlier of June 30, 2025 or until terminated by either party.
2. The Utah Department of Health & Human Services Contract Amendment
This Contract is effective July 1, 2023, by and between the Utah Department of Health & Human Services (Department) and Weber Human Services (Contractor), purpose of contract amendment: New attachments B, C, D and E, effective 7/1/2023, are replacing attachments B, C, D and E, which were effective 7/1/2022.
3. Memorandum of Understanding between Weber Human Services and Fremont5 CTC.
This Memorandum of Understanding (MOU) is made effective as of July 1, 2023, between Weber Human Services, a local substance abuse authority and Fremont5 CTC, whose fiscal agent is Weber Human Services. This agreement is intended to outline the roles and responsibilities of the partnership and to increase the capacity for evidence base substance use prevention services, programs, and strategies in the Fremont 5 CTC service. It is the intent that this collaborative effort ensures that specified activities will best meet the needs of children and families, as feasible, based on eligibility guidelines and available funding. Distribute annual disbursement of up to $5,000 (non-match) to the coalition as long as funding is available.
4. Memorandum of Understanding between Weber Human Services, Roy Communities That Care, and Ogden-Weber Community Action Partnership.
This Memorandum of Understanding (MOU) is made effective as of July 1, 2023, between Weber Human Services, a local substance abuse authority and Roy Communities That Care, whose fiscal agent is Ogden-Weber Community Action Partnership (OWCAP). This agreement is intended to outline the roles and responsibilities of the partnership and to increase the capacity for evidence base substance use prevention services, programs, and strategies in the Fremont 5 CTC service. It is the intent that this collaborative effort ensures that specified activities will best meet the needs of children and families, as feasible, based on eligibility guidelines and available funding. Distribute annual disbursement of up to $5,000 (non-match) to the coalition as long as funding is available.
5. Agreement #A05-2023, Interlocal Cooperation Agreement
This Agreement is made pursuant to the provisions of the Interlocal Cooperation Act, U.C.A. 11-13-1 and by and between North Ogden City, hereinafter called the 'City' and Weber Human Services, a political subdivision of the State of Utah, hereinafter called 'WHS', the purpose of this Agreement is for providing adequate facilities and services to meet the needs of the senior citizens in the northern part of Weber County. Commencing on July 1, 2023 and ending June 30, 2024. This agreement may be terminated at any time by either party upon six months prior written notice to the other party.
6. Agreement between Weber Human Services and Renew Home Health for the provisions of Home and Community Based Services
This Agreement made by and between Weber Human Services, an Interlocal Entity and political subdivision of the State of Utah, hereinafter referred to as 'WHS' and Renew Home Health, hereinafter referred to as 'Provider', whereas the parties hereto desire to affiliate in providing home and community based services to the adults (age 18 or over) of Weber and Morgan Counties; and the parties hereto have determined that the provisions of this agreement are mutually beneficial to both. The terms of this Agreement will be for a period of 24 months commencing on the 1st day of July, 2023 and terminating on the 30th day of June, 2025 unless terminated sooner by either party, which termination may be without cause by giving 30 days written notice to other party.
