Description/Agenda

INVITATION for PROPOSALS Naples City is soliciting proposals for the re-construction of Approximately 1300 sq ft of roadway located at the intersection of 2850 South 1500 East. The purpose of this project is to create smoother transitions to the Highway through the removal of the waterway. The Contractor will be responsible for the removal of the existing concrete water crossing, curb, gutter, asphalt, and base materials for the project. After the removal of those items the contractor will replace the base, asphalt (making sure to maintain the crown of the road to match existing), curb, and gutter. Approximately 860 square feet of asphalt and base will be replaced and approximately 60 linear feet of curb and gutter, with an added option to replace approximately 210 square feet of sidewalk. This work shall consist of furnishing all labor, material, and equipment necessary to perform all operations for the re-construction of the specified roadway. An approved traffic control plan will be required. All work shall be in accordance with Naples City's specifications and standards. Construction shall be completed before November 15, 2022. The road shall not be closed for more than 7 days, if the project runs longer than the 7 days 1 lane must be kept open for fire equipment exit and entrance. The total project duration is not expected to exceed 14 calendar days from the start of construction. Each Proposal must include a description of work to be performed, at least 2 references, and price for the project, including the cost for the optional sidewalk replacement item (approximately 210 square feet). Each Proposer must be able to provide their insurance policy, W-9 form, business license, and workers compensation documentation upon request. The party to whom the contract is awarded will be required to obtain the necessary Road Encroachment permit from Naples City, as well as be required to obtain a PERFORMANCE BOND and PAYMENT BOND within ten (10) calendar days from the date when the NOTICE OF AWARD is given. Sealed proposals will be accepted until Thursday, September 22, at 12:00 PM by mail or delivery to Mike Davis, Naples City, 1420 E. 2850 S. Naples, UT 84078. Proposals must be in a sealed envelope and labeled '2850 S. Construction Project'. Late proposals will not be accepted. Proposals will be opened at 12:00 PM and held for evaluation by the City, and authorization of the City Council. Naples City's evaluation process will be based upon price (50%), description of work to be completed (25%), and references (25%). Naples City reserves the right to accept or deny any or all proposals for and waive any information or technicality on any proposal in the interest of the City and their decision will be final.
