a) Request to Ratify the Contracts as Presented
1) Addendum to MOU Between Greenwood Charter School and WHS
This Agreement is between Weber Human Services (WHS) and Greenwood Charter School. This MOU is made effective as of August 24, 2021. The Addendum is made effective June 1, 2022 until the 1st school day of 2022-2023 school year. The addendum addresses WHS providing additional summer services. This includes providing a masters level clinician for up to 16 hours a week, providing ongoing training and monitoring of clinical services, providing access to electronic charting and supports, and providing quarterly billings and service descriptions. The total cost for services provided up to but not exceeding the original school year amount of $35,000 annually.
b) Request to Approve the Contracts as Presented
1. Morgan City Lease Agreement
This lease agreement is between Morgan City Corporation (Lessor) and Weber Human Services (WHS) (Lessee). This agreement is made effective pending the date of occupation. WHS agrees to pay Lessor rent in the amount of $1,500.00 per month on the first day of the first month of occupancy and on the first day of each month thereafter during the Lease term.
2. Almost Anything Remodeling
This Agreement is between Weber Human Services (WHS) and Almost Anything Remodeling (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Handyman (per hour) $95.00 with a minimum service call of $150.00.
3. Mountain View Health Services
This Agreement is between Weber Human Services (WHS) and Mountain View Health Services (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Adult Day Care (full day) $178.00/$199.00, Adult Day Care (half day) $7.50, Emergency Response System (see attached rate sheet.).
4. The Grandkids LLC.
This Agreement is between Weber Human Services (WHS) and the Grandkids LLC. (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: companionship service (per hour) $27.00, Homemaker (per hour) $27.00.
5. Daybreak Senior Services
This Agreement is between Weber Human Services (WHS) and Daybreak Senior Services (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Adult Day Care (Full Day) $79.00, Adult day Care (Half Day) $49.00, Assisted Transportation (one way trip or per mile) $15.00.
6. Acorn Care LLC. Dba Homewatch Caregivers of Utah
This Agreement is between Weber Human Services (WHS) and Acorn Care LLC. Dba Homewatch Caregivers of Utah (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Chore Service (per hour) $35.00, Companionship Service (per hour) $27.00, Homemaker (per hour) $27.00, Personal Care (per hour) $27.00, Respite (hourly) $27.00.
7. Visiting Angels
This Agreement is between Weber Human Services (WHS) and visiting Angels (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted Transportation (one way trip or per mile) $.59/mile, Companionship Service (per hour) $30.00, Homemaker (per hour) $30.00, Personal Care (per hour) $34.00, Respite (hourly) $30.00, Respite (overnight) $34.00.
8. Suzy's Senior Companionship Services
This Agreement is between Weber Human Services (WHS) and Suzy's Senior Companionship Services (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted Transportation (one way trip or per mile) $10.00, Chore Service (per hour) $25.00, Companionship Service (per hour) $24.00, Homemaker (per hour) $24.00, Nurse service (per hour) $70.00, Personal Care (per hour) $24.00, Respite (hourly) $24.00.
9. SDX Homecare Operations LLC.
This Agreement is between Weber Human Services (WHS) and SDX Homecare operations LLC. (Provider). The parties hereto desire to affiliate in providing home and community-based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1, 2022 and terminating on June 30, 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted Transportation Services (one way trip or per mile) $1.00, Chore service per hour) $28.00, Companionship (per hour) $28.00, Emergency Response System (set-up free) $50.00, Emergency Response System (one month) $50.00, Homemaker (per hour) $28.00, Personal Care (per hour) $30.00, Respite (hourly) $28.00, Respite (overnight) $30.00.
