Sandy Suburban Improvement District ('District' or 'SSID') is Seeking a contractor for on-call repair and maintenance work and for future sewer line replacement projects. SECTION 1- OVERALL SERVICE DESCRIPTION AND REQUIREMENTS The District would like to select a contractor for a duration of 3 years. The contractor will perform on- call (24:7) repair and maintenance work for the District, which includes, but is not limited to, pipes, manholes, clean-outs, and other such work as may be mutually agreed upon by the District and Contractor. The contractor will also complete between one to two projects per year (depending upon SSID budgets) of sewer pipeline replacements/upgrades between 800 and 1,200 linear feet per project. It is anticipated that a written contract between the Contractor and the District will be subject to renewal for up to two additional 1-year extensions dependent upon mutual consent of all parties involved. The selected contractor will also perform all work in accordance with drawings, plans, and specifications referenced in SSID 'work orders' and prepared by SSID's engineer. In addition, all plumbing codes and construction standards, applicable for work in either Sandy City or Salt Lake County, or within UDOT rights-of-way are to be complied with. The qualification process will be based on the point system described in Section 3. Due Date Statements of Qualification are due no later than 4:00 PM, Local Time, on June 16th, 2022 at the Sandy Suburban Improvement District Offices located at 8855 S 700 W, Sandy UT 84070. No submittals will be accepted after the date and time indicated. Contact Information Sandy Suburban Improvement District will not be held responsible for any oral instructions. Any changes to this Request for Statement of Qualifications will be in the form of a published amendment. All technical questions regarding this Request for Statement of Qualifications must be submitted in writing through email to:Jared@JWOEngineering.com SECTION 2- THE SCOPE OF WORK FOR THE PROJECT WILL INCLUDE: A. ON-CALL REPAIR AND MAINTENANCE a. 24:7 services with the response time being no more than 30 minutes from phone call or electronic notification and equipment on-site within 120 minutes. b. Timely restoration of sewer flow interrupted by blockages c. Timely repair and maintenance of pipes, manholes and clean-outs d. Repair and maintenance for other work mutually agreed upon from time to time B. PIPELINE REPLACEMENT AND UPGRADES a. Replacement of sewer pipelines b. Upgrades to sewer pipelines c. Other projects as deemed necessary by the District. SECTION 3- EVALUATION CRITERIA Each Statement of Qualification submitted will be evaluated through a qualifications-based evaluation process only. Statements of Qualification will be scored on the basis of 100 total possible points as follows: Qualifications Evaluation Criteria A. Contractor Capabilities (10 points) Discuss the capabilities of the contractor and identify base of operation location(s) that will provide the services outlined in this RFP. Outline type and amount of equipment owned or leased by the Contractor. Also provide information on your company's bonding capacity. How long has your company been with a bonding company? And, have others (or your company) ever had to file on one of your bonds? B. Past Project Experience (30 points) Discuss the past project experience of your company with On-Call Sewer Services, pipeline installation, replacement projects in residential areas, and knowledge, if any, of the SSID collection system . Demonstrate ability of your company to meet the experience requirements discussed herein. Please provide the following reference information for at least (3) previous projects, similar to those discussed, within the last (5) years: 1. Brief description of the project 2. Role of your company in the project 3. Project Owner 4. Reference Information (one current name and telephone number per project) *Note: One negative response from a reference will result in a 5-point deduction. Two negative responses will result in 0 points on this line item. C. Key Personnel and Recent Relevant Experience (15 points) Describe experience and qualifications of the specific key personnel to be assigned to these services. For each key person identified, list their length of time with your company, a description of their role in the project, and specific similar projects for which they played a similar role. D. Project Management Procedure (10 points) Submit how your company manages projects and identify each team member's role in the services to be provided. Also, provide an organization chart of key personnel. Contractor should address: (i)how it interacts with the engineer, owner, and public; (ii) How it manages projects to ensure the project gets completed within schedule, budget; (iii) how it ensures its quality of work. E. Overall Evaluation of the Contractor (10 points) Discuss Contractor's ability to meet the response timeframes indicated in section 2 of this RFP. Describe any past experience with meeting similar response requirements. F. Time and Material Rates (20 points) In a separate sealed envelope, submit your T&M rates. List all your equipment, personnel time, and material rates. Each contractor will receive 5 points for just submitting their time and material rates and 0 - 15 points on the quality of the costs. G. Insurance claims. (5 Points) Submit insurance claims within the last 5 years against your company and a brief description of them, if any. SECTION 4- REQUIREMENTS OF STATEMENT OF QUALIFICATIONS Contractors interested in providing these services should submit a written statement of qualification in a sealed envelope with a separate sealed envelope which includes it's time and material rates. The written statement of qualification shall include a one-page cover letter plus a maximum length of 10 pages to address the RFP criteria. Resumes for each key team member shall be limited to one page each and should be incorporated as an appendix at the end of the RFP. The appendix does not count towards the 10-page limit. Resumes need to contain employee information, not additional company information. Also, please provide a separate company profile in the resume appendix. Adherence to the maximum page criterion is critical: each page size (maximum 8-1/2' x 11), one side only, with criteria information will be counted. Pages that have project photos, charts and/or graphs will be counted towards the required number of pages. Front and back covers, Table of Contents pages, and divider (tab) pages will not be counted unless they include qualifications information that could be considered by the selection team. Any additional company information or non-key staff information included in the resume section will be counted against the maximum page requirement. Do not submit additional information not listed herein. Font size may not be less than 12 point. Please be advised that failure to comply with the following criteria may be grounds for disqualification, and will be strictly enforced: - Receipt of submittal by the specified cut-off date and time. - The number of copies of the submittal specified.(3 copies) - Adherence to maximum page requirement. (10 pages not including appendix) - Sealing of the envelope containing the statement of qualifications - Deposit of submittal to correct location. - Not following submittal guidelines in this request.
