Agenda Board of Directors Meeting Weber Human Services 3rd floor Board of Directors Room commencing at 8:00 A.M. February 19, 2021. 1. Consent Calendar: a) Welcome b) Request for approval of minutes for meeting held on December 18, 2020 at 8:00 a.m. c) Request for approval of check register dated December 01, 2020 through December 31, 2020; and check register dated January 1, 2021 through January 31, 2021 including voided checks; 120001, 121251, 121538, 121659, 121726, 121773, 121829, 121709,121821,122002,122033,122106. Covering checks numbered 121881 through 122165 in the amount of $2,229,186.10. Also covering checks numbered 122166 through 122465 in the amount of $1,338,539.48. d) Credit Card Purchases. e) Request to approve and Ratify purchase orders: 1. Ratify PO#4622 to Marsh USA for Liability Insurance renewal for 2021 in the amount of $293,604.00. 2. Ratify PO#4625 to Intermountain Lock and Security for automatic door closers on main doors in the main building in the amount of $8,621.32. 3. Ratify PO#4626 to Ken Garff Nissan of Salt Lake City for 5 new meals on wheels vehicles in the amount of $106,219.75. 4. PO#4628 to TH Bell Jr. High School for 8 halo smart sensor vape detectors for team locker rooms request was made by Principal Lenn Ward. In the amount of $10,360.00. 2. Action Items A) Request to Ratify and approve the Contracts as presented. Memorandum of Understanding between Weber County School District and Weber Human Services This MOU is made effective as of January 1, 2021 by and between Weber Human Services (WHS) as a political subdivision of the State of Utah, with it's principal office located at 237 26th street Ogden, Utah 84401; and Weber County School District (WSD) a political subdivision of the State of Utah, with it's principal office located 5320 Adams Ave. Parkway Ogden UT, 84405. This agreement is for Clinical Services for in the Weber County School District for severe students with behavioral needs. These students will either be paid for for by Medicaid if they have it or throught the early intervention grant. Agreement between Weber Human Services and Connect America.com LLC This agreement made by and between Weber Human Services, an interlocal entity and political subdivision of the State of Utah, hereinafter referred to as 'WHS' and Connect America.com LLC hereinafter referred to as 'Provider'. The purpose of this agreement is for the provision of home and community based services. The term of this agreement will be for a period of 19 months and commencing on December 1, 2020 and terminating on June 30, 2022 unless terminated sooner by either party, which termination may be withoiut cause by giving 30 days written notice to the other party. Services provided by this contract will be providing home and community based services to the adults (age 18 or over) of Weber Morgan Counties. B) Independent Audit Results 3.
