PO#4608 to Judge Baker Children's Center for a five day Clinical Training in the amount of $9,300.00. 2. Action Items A) Request to Ratify the Contracts as presented. 1. State of Utah Grant agreement Amendment Log#38980 Amendment #3 to contract #172132 To be attached to and made a part of the above numbered contract by and between the State of Utah, Department of Human Services Agdency code 200. Referred to as State Entity and Weber Human Services , referred to as contractors. The parties agree to amend the contract as follows: New ending date 06/30/2021 New contract amount $329,779.00 effective date of Amendment 07/01/2020. 2. State of Utah Department of Human Services Contract Amendment Contract#160383 Amendment#49 To be attached to and made a part of the above numbered contract by and between the State of Utah Department of Human Services, which includes the division of Substance Abuse and Mental Health and Weber Human Services. The purpose of this Amendment is to extend the end date. The parties agree to amend the contract as follows replace the end date with July 30, 2020. Effective date of amendment June 30, 2020. All other conditions and terms in the original contract and previous amendments remain the same. 3. State of Utah Department of Human Services Governmental Entity Contract for Services The general purpose of this contract is to provide home delivered meals (meals on wheels), delivery service (grocery, RX assistance, etc.), transportation, expansion of CM services as needed, telehealth, outreach, and other services necessary to keep high-risk individuals for severe illness from COVID-19 safe in their homes. This contract is entered into as a result of a procurement process under Utah code 63G-6a-2103. Effective date May 1, 2020 termination date December 29, 2020 unless terminated early or extended in accordance with the terms and conditions of this contract. Contractor will be paid a maximum of $323,715.00 for costs authorized by this contract. Additional information regarding costs (all funds must be expended prior to December 30, 2020. 4. State of Utah Department of Human Services pass through agreement for services by a local authority DHS agreement#A03084 This pass through agreement is between the State of Utah Department of Human Services, Division of Substance Abuse and Mental Health ('DHS') and Weber Human Services, 237 26th Street, Ogden, UT,84401, a local mental health authority ('LMHA') and local substance abuse authority ('LSAA'), (collectively 'LA') and begins on July 1, 2020 and ends on June 30, 2025. Payments made to the LA will be the LA's total compensation for agreement services provided. Payments will be made via electronic funds transfer (EFT). If the LA is not enrolled EFT, the LA shall contact the DSAMH Administrative Services Director to receive instructions for enrolling. Failure to enroll in EFT may cause payments to the LA to be delayed. The contractor shall promptly notify DHS of changes to electronic funds transfer account information. 5. Agreement between Episcopal Managemnet corporation of Ogden and Weber Human Services This agreement is made effective the 1st day of July 2020 and terminating on the 30th day of July 2021, Either party may terminate this agreement at any time without cause by giving the other party 30 days written notice. The parties hereto are mutually desirous of affiliating to provide limited service coordination services for the residents of Union Gardens. Owner agrees to pay WHS as follows. Service coordination rendered an hourly basis, at the rate of $ 28.60 per hour with a maximum of 30 hours per week or 1560 hours annually, which will include benefits when necessary. Travel reimbursement at $0.575 per mile for othe service coordinator to travel to local trainings and meetings in any designated location. Owner agrees that monthly billings by WHS will be paid on/or before the 20th day of each month following the month that services were rendered. Owner agrees to provide on-site office space, telephone, and needed office supplies. 3. Request to approve the contracts as presented. 1. Interlocal agreement for the Northview Senior Center ROLL CALL VOTE The purpose of this agreement is for providing adequate facilities and services to meet the needs of the senior citizens in the northern part of Weber County. WHS agrees to pay for fifty percent of the directors salary, including benefits. Nutrition program costs. Cost of kitchen help, serving equipment and meals. Cost associated with transportation programs. Health promotion costs. Outreach information costs. The cost of office equipment, and supplies. Telephone expenses of fifty percent of the telephone expenses. City shall pay for fifty percent of the directors salary including benefits. The city agrees that the director shall be an employee of WHS. Remaining costs of telephone services. All costs relating to interior and exterior building maintenance and repair, including landscape maintenance. City shall also pay for and be responsible for snow removal. All costs of utilities except telephone expenses, which shall be paid as, specified above. No other costs shall be covered by the City unless specifically agreed to in writing. Users including North View Senior Citizens Board shall collect and keep their own revenues and pay for all costs related to individual group educational activities. All costs related to individual group recreational activities. All costs related to individual group social activites. All special supplies including but not limited to ceramics, porcelain, paints,etc. 2. Agreement between Weber Human Services and Graham Enterprises This agreement made by and between Weber Human Services, an interlocal Entity and political subdivision of the State of Utah, hereinafter referred to as 'WHS' and Graham Enterprises, hereinafter referred to as provider. The term of this agreement will be for a period of 23 months commencing on the 1st day of August 2020 and terminating on the 30th day of June 2022 unless terminated sooner by either party, which termination may be without cause by giving 30 days written notice to other party. Weber Human Services home and community based services schedule of services and rates, lawn care (weekly mowing) $37.00 per 1/4 acre, Hourly labor (general labor) $35.00 per hour, Sprinkler repair $75.00 plus parts, Fertilizer application bid per square feet, residential snow removal $60.00 per hour. 6.
