Request for approval of check register dated July 1 2019 through July 2019; including voided checks; 115217, 115610, 115859, 115960. a) Request to Ratify the Contracts as presented.
1. State Waiver Contract#160496 Amendment #7
This contract amendment is between the Utah Department of Human Services which includes the Division of Aging and Adult Services (DHS/DAAS) and Weber Human Services (Local Agency). The purpose of this Amendment is to 1) Add the FY2020 Maximum Amount. The total Maximum Amount should be $48,100.00. 2) Add the FY2020 Client Service Budget ($388,000.00). 3) Add the Fy2020 Aging Waiver Slots. All other terms and conditions remain the same. The Amendment is effective July 1, 2019.
2. State Aging Contract #160497 Amendment #10
This contract amendment is between the Utah Department of Human Services which includes the Division of Aging and Adult Services (DHS/DAAS) and Weber Human Services (Local Agency). The purpose of this contract is to add Federal Funds for FY2020 and add the new budget sheet for FY2020. The new contract amount is $2,029,700.00. This Amendment is effective July 1 2019 through June 30 2020.
3. AOT Contract#171719 Amendment #3
This contract is between the State of Utah Department of Human Services which includes the Division of Substance Abuse and Mental Health (DHS/DSAMH) and Weber Human Services (Contractor). The purpose of this amendment is to: 1) Add $29,075 in funding for 09/30/2018 - 06/30/2019; 2) add the attached budget; and 3) replace the funding period table.
4. Episcopal Management Corporation Contract (Union Gardens)
This contract is between Episcopal Management Corporation of Ogden (Owner) and Weber Human Services (WHS). The purpose of this contract is to provide limited service coordination services for the residents of Union Gardens. Service coordination rendered on an hourly basis at a rate of $26.30 per hour with a maximum of 30 hours per week or 1560 annually which will include benefits when necessary. Travel reimbursement of $0.58 per mile for the service coordinator to travel to local trainings and meetings in designated location. This contract is effective on the 1st day of July 2019.
5. Utah Department of Health (Utah Arthritis Program)
This contract is between The Utah Department of Health Arthritis Program (Department) and Weber Human Services (Contractor). The purpose of this contract is to increase participation in the Living Well self-management programs or physical activity classes throughout Contractor's delivery system. Services billed per participant up to a maximum of $16,800.00; $5,400 for the period of July 1, 2019 - June 30, 2020.
6. Veterans Care Agreement
This contract is between U.S. Department of Veterans Affair (VA) and Weber Human Services (Provider). The purpose of this contract is to provide case management and other requested services to veterans as pre-authorized by the VA. Payments for services agreed upon fee schedule. This contract is effective on the 5th day of June 2019.
b) Request to Approve the Contracts as presented.
1. Golden Hours Senior Center
This Agreement is between Ogden City (City) and Weber Human Services (WHS). The purpose of this contract is for Weber Human Services (WHS) to work with Ogden City (City) to provide services to the senior citizens of Ogden City, Weber County and Morgan County, which services shall include nutritional, educational, transportation, health promotion and socialization programs and activities for older individuals. WHS agrees to pay for 50% of the salary, including benefits of the director and support staff as mutually agreed upon by both parties each fiscal year. 50% of the cost of supplies and office telephone expenses. The cost of nutrition services provided by WHS Senior Nutrition Kitchen and coordination and maintenance of equipment used in the delivery of WHS Nutrition Services. The total expenditures of WHS for the services described shall not exceed $30,000.00 per fiscal year. Effective Date of Amendment: 07/01/2019 and ending 30 June 2022.
2. Ogden Regional Medical Center Inpatient Agreement
This Agreement is between Weber Human Services (Provider) Ogden Regional Medical Center (Ogden Regional). Provider contracts with the Utah Department of Human Services, as the public mental health service provider, and with the Utah Department of Health (DOH), Division of Health Care Financing (DHCF) as the sole Medicaid behavioral health contractor in Weber and Morgan Counties as their sole behavioral health services provider. Provider will subcontract with Ogden Regional for inpatient behavioral health services. Provider agrees to pay Ogden Regional $1250 per day for hospitalization of Medicaid eligible clients. Provider will reduce payment of $75 for each inpatient stay based on the required co-pay which should be collected by Ogden Regional directly from the enrollee. This agreement shall be effective beginning January 1 2018 and continue until June 30, 2022 unless otherwise terminated pursuant to the terms of this agreement.
