STATE RECORDS COMMITTEE MEETING Thursday, August 8, 2019 at 9:00 a.m. to 4:00 p.m. Utah State Archives Building 346 S. The order of the appeal hearings may be determined by the Chair. AGENDA HEARINGS: Eric Peterson v. Attorney General's Office (AGO): Requesting access to the denied AGO 2012 UTA investigation report. Rolf Kaestel v. Utah Department of Corrections: Requesting a fee waiver for access to his medical records related to diabetes diagnostics from 1999-2007. Ozwald Balfour v. Utah Department of Corrections: Requesting access to denied Otrack Database records due to lack of specificity in the description of the requested records. Gordon Thomas v. Utah Department of Corrections: Requesting access to current mental health diagnosis and his diagnosis from 1983. LeRoy Wirz v. Sanpete County Sheriff's Office: Requesting access to Incident Case #18071066 records not previously provided. Withdrawn. Smith Hartvigsen v. Bristlecone Water Improvement District (BWID): Requesting access to BWID public records related to the Board members, budget, minutes, operations affecting Garfield County or residents, policies/procedures, resolutions, conflict of interest statements, annexations of district boundaries, etc. Withdrawn.
