1. Utah Department of Health Contract Amendment #172700414
This contract amendment is between the Utah Department of Health (Department) and Weber Human Services (Contractor). The purpose of this contract is to replace Attachment E, Payment Methodology, with a new Attachment E that contains the State Fiscal Year (SFY) 2018 rates and to update Attachments B, C, and D with current federal guidelines. This Amendment is effective 07/01/2017.
2. State of Utah Department of Human Services Contract#160496 Amendment#7
This contract is between the State of Utah Department of Human Services which includes the Division of Aging and Adult Services (DHS/DAAS) and Weber Human Services (Local Agency). The purpose of this amendment is to: 1) Add the FY2020 Maximum Amount. The total FY2020 Maximum amount shall be $48,100.00. 2) Add the FY2020 Client Service Budget ($388,000.00) 3) Add the FY2020 Aging Waiver Slots (38). All other terms and conditions in the original contract remain the same. The Amendment is effective July 1 2019.

1. SSBG (Social Services Block Grant) Contract #172132 Amendment #3
This contract is between the State of Utah Department of Human Services Agency Code 200 (State Entity) and Weber Human Services (Contractor). The purpose of this contract is to amend the contract as follows; New Ending date 06/30/2020. New Contract Amount $82,420.00. Match Requirement; required match amount from $20,626.00 to $20,605.00. Effective Date of Amendment: 07/01/2019.
2. Ol' Pals LLC
This Agreement is between Weber Human Services (WHS) and Ol' Pals LLC (Provider). The parties hereto desire to affiliate in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Chore Services $20 per hour, Companion Services $20 per hour, Homemaker $20 per hour, Respite $20 per hour, Yard Maintenance mower fee $4.00, Yard Maintenance haul clippings fee $4.00.
3. Avamere Home Health Care, LLC.
This agreement is between Weber Human Services (WHS) and Avamere Home Health LLC (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Homemaker $25, Nurse Services $100 per hour, Personal Care $25 per hour, HHA $25.
4. Alertline
This agreement is between Weber Human Services (WHS) and Alertline (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Emergency Response System set up fee $50, Emergency Response System one month $25.00.
5. Comfort Keepers
This agreement is between Weber Human Services (WHS) and Comfort Keepers (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Companionship Service $23.50 per hour, Emergency Response System Set up fee $75, Emergency Response System one month $39.99, Homemaker $23.50 per hour, Personal Care $23.50 per hour, Respite $23.50 per hour, Respite overnight $23.50, Mileage .80 cents per mile, Live-In $350.00.
6. Daybreak
This agreement is between Weber Human Services (WHS) and Daybreak (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Adult Day Care full day $34.95 (Includes lunch), Adult Day Care half day $49.95, Assisted Transportation one way trip or per mile $12.00 (Weber County).
7. Dignity Home Health
This agreement is between Weber Human Services (WHS) and Dignity Home Health (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Companionship Service $23.44 per hour, Homemaker $24.84 per hour, Nurse Service $94.74 per hour, Personal Care $39.82 per hour, Respite $39.82 per hour.
8. Home Health Services of Utah dba At Home Care of Seniors (ATHS)
This agreement is between Weber Human Services (WHS) and Home Health Services of Utah dba At Home Care of Seniors (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted Transportation $10 per 1 way trip, Companionship Service $20 per hour, Homemaker $23.00 per hour, Respite $23 per hour, Transportation (lift van) $15.00 per one way trip.
9. Home and Family Care Services
This agreement is between Weber Human Services (WHS) and Home and Family Care Services (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Chore Services $24.00 Davis/Weber Counties (2 hours or less $48 or $24 per hour), Companionship Service $24.00 Davis/Weber Counties, Homemaker $24 per hour Davis/Weber Counties, Transportation $24 per hour plus .565 added per mile (Weber County), Cleaning rain gutters $62 per hour (Weber County).
10. Home Helpers of Northern Utah
This agreement is between Weber Human Services (WHS) and Home Helpers of Northern Utah (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted Transportation .56 per mile or IRS rate, Chore Service $24 per hour, Companionship Service $24 per hour, Emergency Response Team $35 per month (plus $15 for cellular), Homemaker $24 per hour, Personal Care $24 per hour, Respite $24 per hour (plus $3 hour non-sleep time), Holiday Rates: time and a half, double time for Christmas and Thanksgiving.
11. Mytrex Inc. dba Rescue Alert
This agreement is between Weber Human Services (WHS) and Mytrex Inc. dba Rescues Alert (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Emergency Response System set up fee $30.00, Emergency Response System $24.99 one month, RA Minder $20 (2 daily calls), Medication Dispenser with personal emergency response $79.00, Cell based Emergency Response System set up $30, Cell based Emergency Response Monthly $29.99, Mobile Emergency Response System set up $40, Mobile Emergency Response Monthly $45, Fall detection pendant monthly (no extra set up) $5 (must have personal emergency response system).
12. Platinum Venture Group
This agreement is between Weber Human Services (WHS) and Platinum Venture Group (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Price per item, pricing is determined per item.
13. Red Rock Medical Supply
This agreement is between Weber Human Services (WHS) and Red Rock Medical Supply (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Medical Supplies; pricing determined per item Weber/Morgan Counties.
14. Right at Home
This agreement is between Weber Human Services (WHS) and Right at Home (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted transportation $14.94 one way trip, Companionship Service $24 per hour, Homemaker $24 per hour, Nurse Service $80 per hour, Personal Care $24 per hour, Respite (overnight) $350.
15. Visiting Angels
This agreement is between Weber Human Services (WHS) and Visiting Angels (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Assisted Transportation $0.56 per mile (must be active client), Companionship Service $25 per hour, Emergency Response System set up fee $25, Emergency Response System one month $40, Homemaker $25, Personal Care $29 per hour, Respite $25 per hour, Respite overnight $29.
16. Homewatch Caregivers
This agreement is between Weber Human Services (WHS) and Homewatch Caregivers (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Chore Service $27 per hour, Companionship Service $24.50 per hour, Homemaker $24.50 per hour, Nurse Service (RN) $79.04 per hour, Personal Care $26 per hour, Respite $23 per hour, Respite overnight $21.50.
17. Aramere at Mountain Ridge
This agreement is between Weber Human Services (WHS) and Aramere at Mountain Ridge (Provider). The parties hereto desire in providing home and community based services to the adults (age 18 and over) of Weber and Morgan Counties. This agreement will be for a period of 36 months commencing on July 1 2019 and terminating on June 30 2022. WHS will pay provider for authorized services as approved in the contract. The rates for this contract will be as follows: Adult Day Care full day $15 per hour, Adult Day Care half day $15 per hour, Respite overnight $175 night AL $200 night Memory Care.
