Agenda Board of Directors Meeting Weber Human Services 3rd floor Board of Directors Room commencing at 8:00 A.M. Friday, March 15th, 2019. 1. Executive Session: a) Motion to close and open meeting for the purpose of discussing the character and/or competence of an individual. 2. Consent Calendar: a) Welcome b) Request for approval of minutes for meeting held on February 15th, 2019. c) Request for approval of check register dated February 1 2019 through February 28th 2019; including voided checks; 113691. Covering checks numbered 113957 through 114401 in the amount of $1,798,401.84. d) Credit Card Purchases. e) Request to Approve Purchase Orders: 1. PO #4493 to National CineMedia for advertising at Tinseltown Newgate starting 5/31 2019 for 5 weeks (14 Screens) in the amount of $3,000.00. 2. PO #4494 to Johnson Mechanical Solutions LLC for Emerson sf High Efficiency Unit Freezer Condenser (roof unit), Installation, and power to roof in the amount of $9,717.00. 3. PO #4495 to Larry H Miller Sports and Entertainment for Pre-Feature Commercials (Junction Ogden) May 31st 2019 - June 27 2019 in the amount of $2,480.00. 4. PO #4496 to Superior Computers for 3 Intel Core i5-8600K Coffee Lake Processor in the amount of $2,790.00. 5. PO $4501 to Sam T Evans for LEER DCC, Installation, Tool Box, LED Lighting, Weather Guard Pac Rat in the amount of $3,610.00. 3. Action Items a) Request to ratify contracts as presented. 1. LSI Business Development This contract is between LSI Business Development, Inc. and Weber Human Services (WHS). The purpose of this contract is to provide strategy services for the integration of WHS Prevention and Prosperity initiative to align with the Weber County IGP (Intergenerational Poverty Advisory Commission) initiative. LSI will facilitate, align and support Weber Human Services strategic integration of prevention programs with the Weber Morgan Health Department and the Weber County Intergenerational Poverty Initiative. LSI will focus on integrating diverse assets and subject matter expertise among community stakeholders this agreement shall remain in effect until March 30, 2021. This agreement may be renewed for an additional 3-year period if agreed upon in writing by both parties. LSI shall be compensated at a base rate of $15,000.00 a period of four months, beginning March 1 2019 - June 30, 2019. Services shall be invoiced at $3,750.00 per month for the contract term. 2. Center of Excellence Contract: This Contract is not written at this time but after speaking with the Weber County Commission, they wanted the WHS board to vote on concept as an action item. Weber County Commission will continue to commit the $60,000 amount that is in the LSI contract towards the Center of Excellence (COE) to hire a center director. Weber Human Services and the Weber/ Morgan Health Dept. will each contribute $47,500 towards this position. The COE will be a central hub for any organization wanting to belong to it. The standards of the COE will include that any organization belonging to it will prescribe to Evidence Based Practices in their treatments and programs that are offered. The COE will strengthen agencies to collaborate more closely, use community assessments to determine direction for community residents of Weber County. Its purpose will also be to bridge businesses into being engaged as partners through the Chamber of Commerce and will encourage community residents and organizations who treat, educate or provide any kind of service to the community, to belong to Community coalitions that will be centered around High Schools of both the Ogden and Weber S.D.'s. The umbrella organization will strengthen Weber County as it will prioritize and apply for funding on a local, state and national level and will collectively determine how funding obtained will be used to benefit those receiving services. b) Request to approve contracts as presented. 1. Medicaid Waiver Amendment Contract #160496 Amendment #6 This contract is between the State of Utah Department of Human Services, which includes the Division, Office or unit of the Utah Department of Human Services ('DHS'), and Division of Aging and Adult Services ('DHS/DAAS') and Weber Human Services Weber Area Agency on Aging (Local Agency). The purpose of this Amendment is to 1) Increase the FY2019 Administrative Services Budget Amount $5,000.00. The Approved Budget Amount will be $44,200.00 and the Discretionary Amount will be $7,900.00 for a Total Maximum Budget Amount of $52,100.00. Effective date of the Amendment, February 20, 2019. All other conditions and terms in the original contract and previous amendments remain the same. c) WHS Federal Awards Policy and Procedure (Second Reading) d) Purchasing Policy (Second Reading) 4.
