1. Consent Calendar:
a) Request for approval of minutes for meeting held on August 17th, 2018.
b) Request for approval of check register dated August 1, 2018 through August 31st, 2018 including voided checks; 107672, 108987, 109639, 110371, 110492,110973; covering checks numbered 111523 through 111966 in the amount of $1,730,116.35.
c) Credit Card Purchases.
2. Request to Approve Purchase Orders:
1. PO #4441 to SHI Technologies for Mimecast M2A, Add On-Secure Messaging, LCS-Sliver in the amount of $15,945.00.
2. PO #4443 to Leaders247 for Leadership Development program for WHS focusing on Executive Level Leadership and Strategy Development in the amount of $16,000.00.
3. PO #4444 to Brian C. Miller for Implementation of CE-CERT. Includes Training and Consultation on supervisory processes to sustain implementation in the amount of $5,700.00.
4. PO #4446 to Utah Transit Authority for 20% Match for 2018 Ford Transit Accessible Van; UTA Contract #17-2143JH Grant 5310 in the amount of $12,537.16.
5. PO #4447 to Ogden Lawn & Garden for HP Lawn Mower, 48'Mulch Kit in the amount of $3,738.42.

a) Request to ratify contracts as presented
1. Utah Department of Health Contract
This contract is between the Utah Department of Health (Department) and Weber Human Services (Contractor). The general purpose of this contract is to provide primary health care services to medically underserved populations. The service period of this contract is 07/21/2018 through 06/30/2019 unless terminated or extended by agreement in accordance with the terms and conditions of this contract. The Department agrees to pay $21,000.00 in accordance with the provisions of this contract.
2. AOT Contract#171719 Amendment #2
This contract is between the Utah Department of Human Services (DHS/DSAMH) and Weber Human Services (Contractor) to replace the funding period rate table. 7/1/18-9/29/18 for $66,058.00 and 9/30/18 - 6/30/18 for $225,000.
3. State Youth Treatment Implementation Grant Contract#A02495 Amendment #1
This contract is between the State of Utah Department of Human Services (State Entity) and Weber Human Services (Contractor). The purpose is to update contract period and amount. 7/1/18 - 9/30/18 for $9,949.83 and 10/1/19 - 6/30/19 for $35,400.00.
4. State Aging Contract#160497 Amendment #7
This contract is between the Utah Department of Human Services (DHS/DAAS) and Weber Human Services. The purpose of this Amendment is to add Federal Funds for FY2019 and add the new budget for FY2019. The new FY2019 Contract Amount of $1,847,836.00.
b) Approve contracts as presented
1. Weber/Morgan Children's Justice Center
An Interlocal Agreement by and between Weber/Morgan Children's Justice Center (CJC) and Weber Human Services (WHS) to provide mental health services. WHS shall bill a rate of $80 per therapy hour of 50 minutes. Effective as of date of execution of this agreement by both parties.
2. Episcopal Management Co.
This agreement is between Episcopal Management Corporation of Ogden, Inc. (Owner) and Weber Human Services (WHS). Made effective the 1st day of July 2018 expiring June 30 2019. WHS agrees to provide a service coordinator to Union Gardens for $24.26 per hour max of 30 per week.
3. Autism Preschool Contract#142129 Amendment#5
This contract is between the Utah Department of Human Services (DHS/DSAMH) and Weber Human Services 1) To add funding for SFY 2019; 2) Replace the rate table; 3) Add the SFY2019 DHS Budget Statement Form. 7/1/18 - 6/30/19 for $443,269.00.
4. Allies with Families
This contract is between Allies with Families (Contractor) and Weber Human Services (LMHA) on the Family Resource Facilitator Project beginning July 1, 2018 through June 30, 2019. Contract amount $214,649.
