DOCUMENT 00030 REQUEST FOR BIDS Owner: Kamas City Corporation Address: 170 North Main, Kamas Utah 84036 SEALED BIDS for the Construction of the Kamas City - 2018 90 East CDBG Water Improvements Project will be received by Kamas City at 170 North Main, Kamas, Utah until 1:00 p.m. MDST, September 5, 2018 and then publicly opened and read aloud. PROJECT DESCRIPTION: Replacing approximately 500 lineal feet of 6' waterline with a 10' Ductile Iron Pipe waterline along with valves, fire hydrants, services, fittings, pulverizing & repaving approximately 17,300 square feet of asphalt, remove and replace ~ 990 lineal feet of rolled gutter and remove and replace ~3,900 SF of sidewalk. This project is funded in whole or in part by a grant through the Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) program. The Contractor will be required to comply with all federal labor standards and attendant laws, including the payment of the most current Davis-Bacon wages and compliance with Section 3 to provide employment opportunities for lower income persons and small businesses. Local, minority and woman business owners are encouraged to bid. The lowest responsible bidder will be selected. The CONTRACT DOCUMENTS, consisting of Request for Bids, Information for Bidders, Information Required of Bidder, Status Verification, Measurements and Payments, Certificate of Non-Collusion, Bid, Bid Bond, Agreement, Payment Bond, Performance Bond, Notice of Award, Notice to Proceed, Change Order, General Conditions, Special Provisions, Specifications, Drawings, and Addenda, may be examined at the following locations: HORROCKS ENGINEERS, 728 West 100 South, Heber, Utah 84032 Copies of the CONTRACT DOCUMENTS may be obtained at the office of HORROCKS ENGINEERS LOCATED AT 2162 West Grove Parkway, Suite 400 Pleasant Grove, Utah 84062, and HORROCKS ENGINEERS LOCATED AT 728 West 100 South, Heber, Utah 84032 upon payment of $30.00 for each set, none of which will be refunded. Date of availability is August 27, 2018. PRE-BID MEETING: 10:00 a.m. MDST, August 30, 2018 Kamas City, 170 North Main, Kamas, Utah 84082 DOCUMENTS REQUIRED AT BID OPENING: 1. Bid forms provided. (Sections 00200, 00300, and 00355) 2. Cashier or Certified check or bid bond made payable to Kamas City Corporation in an amount equal to at least five percent (5%) of the total amount of the bid. Bid bonds must be underwritten by a Surety Company approved by the U.S. Department of the Treasury. (Circular 570, latest edition). DOCUMENT 00210 REQUIRED 24 HRS AFTER BID OPENING. If all required documents are not provided, the bid may be disqualified.
