- Request for Qualification Kearns Oquirrh Park Fitness Center (KOPFC) is seeking statements of qualification and proposals from highly qualified roofing contractor to replace portions of the roof at 5624 S Cougar Lane (4800 West), Kearns, UT 84118. Interested contractors may contact Kelly Briggs, Physical Facilities Manager, at 801-545-4110. Plans and specifications will be available June 30, 2016 from 10:00 a.m. to 5:00 p.m. These may be picked up at 5624 S Cougar Lane (4800 West), Kearns, UT with a $100.00 refundable deposit (upon return of documents) payable to the Kearns Oquirrh Park Fitness Center. Contractors will be required to attend a mandatory pre-bid meeting held on site July 7, 2016 at 10:00 a.m. Proposals are due July 14, 2016 at 2:00 p.m. and must be accompanied by a 5% bid bond. A 100% material and labor bond will be required prior to award. Any requests for substitution must be made in writing to Jim Rohbock, JSR Services, LLC 95 E. Fort Union Blvd. Midvale, UT 84047 or jim@jsrservices.com before the pre-bid meeting.
