Agenda Items: I. Call to Order II. Roll Call III. Pledge of Allegiance: By Invitation IV. Minutes: April 7, 2016 Regular Meeting 1. Rocky Mountain Care requests Commercial Development Approval for a Rocky Mountain Care - Skilled Nursing Facility to be located at the Southeast Corner of 500 East and 1200 South 2. Leadership Circle LLC requests Final Commercial Development Approval for a Specialty Grocery to be located at 989 South Main Street 3. Witt Excavating requests Final Commercial Development Approval for Witt Warehouse to be located at approximately 2805 South Highway 40 4. Discuss and consider recommendation for proposed Form Based Code and Design Criteria 5. Administrative Items: V.
