NOTICE OF FORFEITURE To anyone claiming an interest in certain property ('The Property') described as: i. $3,288.12 United States Currency, in Deseret First Credit Union, Account No. *****2422 naming Jason Payne as an owner/interest holder and located in Salt Lake County, State of Utah; ii. $68,298.15 United States Currency, in Deseret First Credit Union in Acct. No. *****6571 naming AN Distribution as owner/interest holder and located in Salt Lake County, State of Utah; iii. $3,183.74, United States Currency, in Acct. No. ***4374, in Bank of American Fork, naming TJW Corporation as owner/interest holder and located in Salt Lake County, State of Utah; iv. $600.00 United States Currency in Bank of American Fork, Acct. No. ***6567 naming Tamara Ann Birrenkott and Timothy J. Woodward as owners/interest holders and located in Salt Lake County; v. $1,518.55, United States Currency in Bank of American Fork, Acct. No. ***6450, naming Timothy J. Woodward as owner/interest holder and located in Salt Lake County, State of Utah; vi. $1,887.81 United States Currency in Bank of American Fork, Acct. No. ***9191, naming TJW Corporation as owner/interest holder and located in Salt Lake County, State of Utah; vii. $547.78 United States Currency, in Bank of American Fork, Acct. No. ***2063, naming Bam Bamm as owner/interest holder and located in Salt Lake County, State of Utah. On December 4, 2015, the Honorable Katie Bernards-Goodman, Third Judicial Court Judge for Salt Lake County, State of Utah, issued an order in case no. 120902298 forfeiting The Property to the State of Utah. Any claim you may have to the Property will be waived if you fail to petition the court according to the procedures outlined in Utah Code Ann. § 24-4 et seq. within thirty (30) days of this notice.
