1. INTRODUCTION
North Davis Fire District (NDFD) is soliciting proposals from qualified emergency medical service (EMS) providers for the delivery of comprehensive Emergency Medical Services within the North Davis Fire District service area.
The intent of this Request for Proposals (RFP) is to identify a qualified provider capable of delivering high-quality, cost-effective, and accessible EMS, including 911 emergency response, in accordance with all applicable federal, state, and local requirements.
This RFP is being conducted as part of NDFD's review of its EMS delivery system and in accordance with the requirements of Utah Senate Bill 215.
The evaluation will consider the three primary areas identified by SB 215:
- Cost
- Quality
- Access
NDFD currently provides 911 emergency medical services within its service area and is evaluating the current and alternative methods of providing these services.
2. SERVICE AREA
The selected provider shall provide EMS services within the geographic boundaries served by the North Davis Fire District.
The NDFD service area includes:
- City of Clearfield
- City of West Point
- City of Sunset
- Unincorporated Davis County
- MIDA
The service area includes residential, commercial, industrial, and transportation areas and is served by NDFD fire stations and emergency response resources.
The proposer shall demonstrate its ability to provide reliable EMS coverage throughout the entire service area.
3. SCOPE OF SERVICES
The selected provider shall provide a comprehensive EMS system including, at a minimum, the following:
3.1 911 Emergency Response
- 24/7 emergency ambulance availability;
- Advanced Life Support (ALS) capability;
- Basic Life Support (BLS) capability;
- Integration with NDFD fire suppression and first-response resources;
- Coordination with local dispatch;
- Participation in mutual and automatic aid;
- Response to multiple simultaneous incidents;
- Response to major emergencies and mass-casualty incidents; and
- Compliance with all applicable Utah EMS licensing requirements.
3.2 Patient Care
- Care delivered under approved medical direction;
- Compliance with applicable Utah EMS protocols;
- Appropriate credentialing of EMS personnel;
- Quality assurance and quality improvement;
- Electronic patient care reporting; and
- Compliance with applicable state and national EMS data standards.
4. SYSTEM STATUS AND DEPLOYMENT REQUIREMENTS
Proposers shall describe their deployment model and demonstrate how the proposed system will maintain continuous EMS readiness throughout the NDFD service area.
The proposal shall address:
- Number and type of ambulances available;
- Minimum staffing levels;
- ALS and BLS staffing;
- Paramedic availability;
- Ambulance deployment locations;
- Unit availability;
- Coverage during peak demand;
- Coverage during hospital delays;
- Coverage during simultaneous calls;
- Mutual aid and automatic aid;
- Reserve ambulance availability; and
- Procedures for maintaining adequate coverage throughout the service area.
5. PERFORMANCE STANDARDS - QUALITY AND ACCOUNTABILITY
Proposers shall describe how they will meet or exceed the following performance expectations.
5.1 Response Time Standards
Proposers shall provide historical response-time data for comparable EMS systems and identify proposed response-time standards for the NDFD service area.
The proposal shall identify response-time performance by priority level and explain how performance will be measured.
5.2 Clinical Quality
The provider shall maintain a formal QA/QI program addressing, at a minimum:
- Cardiac arrest;
- Airway management;
- Trauma;
- Medication administration;
- Patient documentation;
- Other high-acuity clinical conditions;
- Clinical education; and
- Medical oversight.
The provider shall identify the clinical performance measures it uses to monitor patient care.
5.3 Reporting
The provider shall provide regular performance reports to NDFD.
Reports should include, at a minimum:
- Response times;
- Unit availability;
- Call volume;
- Staffing;
- System performance;
- Patient care quality measures;
- Other performance measures reasonably requested by NDFD.
6. COST AND FINANCIAL EXPECTATIONS
Proposers shall clearly outline the financial structure of the proposed EMS system.
The proposal shall address:
6.1 System Cost
- Cost to NDFD, if any;
- Any required subsidy or financial support;
- Proposed contract costs;
- Cost containment strategies; and
- Long-term financial sustainability.
6.2 Patient Billing
Proposers shall describe:
- Ambulance transport rates;
- Mileage rates;
- ALS and BLS rates;
- Billing practices;
- Insurance billing;
- Medicare and Medicaid billing;
- Uninsured patient policies;
- Financial hardship policies; and
- Collection practices.
All proposed rates shall be clearly identified.
6.3 Financial Sustainability
The proposer shall demonstrate its ability to maintain a financially sustainable EMS system throughout the contract term.
The proposer shall identify any anticipated annual rate increases or other changes in the financial structure of the system.
7. ACCESS TO SERVICE
Proposers shall demonstrate how they will ensure reliable and timely access to EMS throughout the NDFD service area.
This shall include:
- 24/7 ALS availability;
- Adequate ambulance availability;
- Appropriate staffing;
- Coverage during periods of high call volume;
- Coverage during hospital delays;
- Coverage during simultaneous calls;
- Mutual aid and surge capacity;
- Appropriate ambulance deployment;
- Continuity of service; and
- Procedures for maintaining coverage throughout the service area.
8. ORGANIZATIONAL QUALIFICATIONS
Proposers shall provide:
- Organizational structure;
- Ownership/governance structure;
- Staffing model;
- EMS certification levels;
- Number of ambulances operated;
- Experience providing 911 EMS services;
- Experience serving similar communities;
- Training programs;
- Medical direction;
- QA/QI program; and
- Relevant regulatory or licensing history.
Proposers shall provide at least three references from organizations for which they currently provide or have previously provided EMS services.
9. GOVERNANCE AND OVERSIGHT
The selected provider shall operate in coordination with and under the oversight requirements established by NDFD and the applicable contracting authority.
Requirements shall include:
- Regular performance reporting;
- Participation in EMS system review meetings;
- Cooperation with NDFD's QA/QI process;
- Compliance with applicable NDFD policies and procedures;
- Compliance with applicable state and local requirements;
- Cooperation with emergency response operations; and
- Participation in system planning and improvement.
The contract term shall align with the applicable EMS licensing cycle and shall be subject to applicable Utah law.
10. PROPOSAL SUBMISSION REQUIREMENTS
Proposals shall include:
1. Executive summary;
2. Organizational overview;
3. Detailed response to each section of this RFP;
4. Description of EMS system design and deployment;
5. Staffing plan;
6. Response-time plan;
7. QA/QI and medical oversight plan;
8. Financial proposal;
9. Proposed patient billing structure;
10. Relevant experience and references;
11. Historical performance data, when available;
12. Transition and implementation plan; and
13. Supporting documentation and data.
The financial proposal shall clearly identify all costs and fees associated with the proposed services.

11. EVALUATION CRITERIA
Proposals will be evaluated based on the following criteria:
Evaluation Category Weight
Cost - Financial impact, rates, billing structure, transparency, and sustainability 30%
Quality - Clinical care, medical oversight, staffing, training, QA/QI, and performance 30%
Access - Coverage, response reliability, ambulance availability, deployment, and IFT availability 30%
Experience and Past Performance - Relevant experience, references, and demonstrated performance 10%
Total 100%
NDFD reserves the right to reject proposals that do not meet minimum qualifications or that substantially deviate from the requirements of this RFP.
NDFD may request clarification or additional information from proposers as necessary to complete its evaluation.
NDFD reserves the right to negotiate final contract terms with the selected proposer, subject to applicable law.

This RFP is intended to comply with applicable requirements of Utah Senate Bill 215, including the statutory review of EMS services based upon cost, quality, and access.
Nothing in this RFP is intended to waive, modify, or supersede any rights or requirements established by Utah law.
