REQUEST FOR PROPOSAL DUCHESNE COUNTY Emergency Medical Services 1. GENERAL Objective/Purpose Duchesne County, Utah, is seeking proposals from qualified service providers for Emergency Medical Services (EMS) throughout the county. The purpose of this RFP includes a review and evaluation of emergency medical services and to secure reliable, professional, and sustainable EMS coverage that ensures the health and safety of residents and visitors pursuant to the provisions of 2025 Senate Bill 215 including but not limited to the provisions of Utah State Code §§ 11-48-103, 53-2d-103, 53-2d-505, 53-2d-604, 53-2d-606.5 and 53-2d-607. 2. TERM The term shall be for a period of four years pursuant to Utah State Code §53-2d-501. The County will honor the statutory first right of the incumbent 911 provider to continue interfacility transports, as required under Utah State Code §11-48-103. 3. WORK REQUIREMENTS / QUALIFICATIONS Expectations 1. Responsible for ground ambulance interfacility transports within Duchesne County as needed. 2. Deliver EMS coverage that is financially sustainable for both Duchesne County and the provider. 3. Maximize use of available state/federal funding, grants, and partnerships. 4. Provide transparent billing practices for patients and payers 5. Minimize unnecessary costs to residents while ensuring continuity of service. 6. Meet or exceed state and national EMS response standards. 7. Employ well-trained, licensed, and certified personnel. 8. Demonstrate strong quality assurance/quality improvement programs. 9. Track and report key performance metrics (response times, patient outcomes, etc.) 10. Ensure patient safety through proper protocols, continuing education, and risk management. 11. Provide 24/7 EMS coverage across all of Duchesne County in both populated and remote areas including Roosevelt, Tabiona, Fruitland, Altamont and Duchesne. 12. Ensure equitable response times for all areas of the county, recognizing rural access challenges. 13. Maintain sufficient vehicles, equipment, and facilities to serve our vast area. Qualifications 1. Proof of licensure and certification to provide EMS services in Utah. 2. Proposed staffing plan and qualifications of key personnel. 3. Experience providing EMS services in rural areas, particularly in Utah. Pricing Interested service providers should submit competitive sealed proposals. The County will evaluate all interested qualified service providers. Insurance Maintain at all times adequate insurance, in which Duchesne County is listed as an additional insured. For private insurers, the County expects at a minimum: - Professional Liability coverage in an amount not less than one million dollars ($1,000,000) for each claim and two million dollars ($2,000,000) aggregate. - Workers Compensation and Employer's Liability coverage. Employer's Liability coverage shall be in an amount not less than one hundred thousand dollars ($100,000) for bodily injury caused by accident and one hundred thousand dollars ($100,000) for bodily injury by disease. Waiver of Workers Compensation Coverage may be considered in lieu of coverage. - Automobile coverage in an amount not less than two hundred thousand dollars ($200,000) and five hundred thousand dollars ($500,000) per occurrence. 4. SUBMISSION GUIDELINES Proposals may be submitted either in a sealed envelope addressed to the Duchesne County Commission care of Melissa Hughes, Commission Executive Assistant at 734 North Center Street (PO Box 270), Duchesne UT 84021, or via email addressed to melissa.hughes@duchesne.utah.gov by September 11, 2026 Submissions must be clearly labeled with 'Emergency Management Services' and must include submitter's name and contact information. Proposals will be considered by the Duchesne County Commission. After proposals have been reviewed, further discussions or interviews may be conducted. It is anticipated that final award of contract will be made in the first available duly noticed Commission Meeting after submissions have been reviewed.
