Public Notice for Proposed Amendments to Health Regulation #5 Food Protection The Salt Lake Valley Board of Health has proposed an amendment to Health Regulation #5, Food Protection. The public comment period will end on Wednesday, April 11, 2012. A copy of the proposed amendment is available for review on the Salt Lake Valley Health Department website at http://www.slvhealth.org/envRegs/proposedRegs.html or during normal business hours at the following locations: Salt Lake Valley Health Department Division of Environmental Health 788 East Woodoak Lane Murray, Utah 84107 Salt Lake Valley Health Department 2001 South State Street Suite 2500 Salt Lake City, Utah 84190 Written comments are preferred and will be accepted if received by 6:00 PM on April 11, 2012. Your written comments should be submitted to: Royal DeLegge, Division of Environmental Health at 788 E Woodoak Lane, Murray, Utah 84107 or by e-mail to slvhdehsan@slco.org. The public hearing on the proposed amendment will be held on April 11, 2012 from 5:00 PM to 7:00 PM in the Environmental Health Tech Center Training Room at the Division of Environmental Health, 788 East Woodoak Lane, Murray, Utah. We reserve the right to close the hearing at 6:00 PM if all present have had the opportunity to offer public comment. In compliance with the Americans with Disabilities Act, individuals needing auxiliary communicative assistance should contact Environmental Health Administration, 385-468-3860 five working days before the hearing.
