I, Scott. E Wells, the duly appointed and acting City Manager of Wellsville City, Cache County,
Utah, do hereby certify that I did, on August 6, 2025, receive the Petition for Annexation and attached
Plat which was filed with the city by Jason and Sharla Blackham, which Petition was accepted by the
Wellsville City Council for further consideration pursuant to Section 10-2-405, Utah Code Annotated,
1953 as amended, by Resolution 2025-08 adopted and passed by the City Council on August 20,
2025. Interested parties may inspect and copy the complete annexation petition and Resolution 2025-
08 during normal business hours at the Wellsville City Offices located at 75 East Main.

withdrawn to the Wellsville City Fire District and the Cache County Sheriff's Office, which are the local
districts providing, respectively, fire protection, paramedic, emergency services, and law enforcement
service, as the case may be depending on the location of the proposed area. (To be determined in
accordance with in UCA 10-2-808(h)&(i))
I hereby certify the Petition and state that I, Scott E. Wells, and City Attorney Seth Tait have
reviewed the referenced Petition for Annexation and have determined that the Petition meets the
requirements of Subsections 10-2-403 (3), (4), and (5) of the Utah Code and I hereby give Notice to
the Wellsville City Council, and Jason & Sharla Blackham, of my Certification of said Petition as
meeting the requirements of the referenced subsections of State law.
