The State of Utah DHHS and Local Public Health Governance Committee Meeting Approve Minutes: January 5th, 2026 Grant Title Executive Committee Review Description State Personal Responsibility Education Program Funding Period: 10/1/25-9/30/28 Dollar Amount: $221,541 Application Due: 1/23/2026 Grant Status Expedited This grant was reviewed and approved by HPAP Exec on January 15th, 2026. The Administration for Children and Families, Family and Youth Service Bureau (FYSB) have renewed the State Personal Responsibility Education Program for one quarter under a continuing resolution. Funding must be used for an evidence-based program designed to educate youth on both abstinence and contraception to prevent pregnancy and sexual transmitted infections, including HIV/AIDS. Additionally, the program must incorporate at least three adulthood preparation subjects: financial literacy, education and career success, parent-child communication, healthy relationships, and healthy life skills. Grant Title Executive Committee Review Description Title V State Sexual Risk Avoidance Education Funding Period: 10/1/25-9/30/27 Dollar Amount: $148,997 Application Due: 1/23/2026 Grant Status: Expedited This grant was reviewed and approved by HPAP Exec on January 15th, 2026. The Administration for Children and Families, Family & Youth Services Bureau (FYSB) has extended the availability of funds under the Title V State Sexual Risk Avoidance Education Program for one quarter under a continuing resolution. The purpose of the SRAE program is to fund states to implement education exclusively on sexual risk avoidance that teaches participants to voluntarily refrain from non-marital sexual activity. Grant Title Executive Committee Review Description School-Based Interventions to Promote Equity and Improve Health, Academic Achievement, and Well-Being of Students Funding Period: 6/30/26-6/29/27 Dollar Amount: $390,000 Application Due: 2/13/2026 Grant Status: Needs Review This grant was reviewed and approved by HPAP during the week of January 9th, 2026. The purpose of this 5-year cooperative agreement is to protect and improve the health and wellbeing of school-age children and adolescents in underserved and disproportionately affected communities. Recipients will use the Whole School, Whole Community, Whole Child (WSCC) model to: establish key partnerships to support school health programs; promote dissemination and implementation of CDC school health guidelines, tools, and resources through Professional Development (PD) and Technical Assistance (TA); and use action planning to implement physical activity, school nutrition, and school health policies, practices, programs, and services within state and local education agencies (LEAs), schools, and out-of-school time (OST) settings. Grant Title Executive Committee Review Description The National Cardiovascular Health Program Funding Period: 6/30/26-6/29/27 Dollar Amount: $880,310 Application Due: 3/4/2026 Grant Status: Needs Review This grant was reviewed and approved by HPAP during the week of January 9th, 2026. This NOFO supports state investments in implementing and evaluating evidence-based and evidence-informed strategies to prevent and manage CVD in select populations in each state. This must be achieved by joining or creating an LC and demonstrating capacity to reduce hypertension and high cholesterol in alignment with specified strategies. The persons of focus are impacted by a high prevalence of CVD, exacerbated by health inequities and disparities, social determinants, such as inadequate health care, education, poverty, and unfair opportunity structures. Grant Title Executive Committee Review Description A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes Funding Period: 6/30/26-6/29/27 Dollar Amount: $850,000 Application Due: 2/26/2026 Grant Status: Needs Review This grant was reviewed and approved by HPAP during the week of January 9th, 2026. This Cooperative Agreement seeks to prevent or delay onset of type 2 diabetes by decreasing risk among adults with prediabetes and improve self-care practices, quality of care, and early detection of complications among people with diabetes. All work supported under this Cooperative Agreement will focus on reducing health disparities and achieving health equity for priority populations, defined as those who have systematically experienced greater obstacles to health. DHHS will work closely with LHDs and other contracted partners to implement a variety of strategies to decrease risk for type 2 diabetes among adults at high risk; improve selfcare practices, quality of care, and early detection of complications among priority populations with diabetes. We would like the Governance committee to confirm this designation before moving forward) Grant Name Application Due Description Pregnancy Risk Assessment Monitoring System 2/16/2026 This is a one-time, non-competing supplement of up to $175,000 for our award under CDC-NOFO RFA-DP-21-001. The supplement will extend our award and be applied to the current budget period 05/01/2025 through 4/30/2026, extending it to up to 04/30/2027. Government Efficiency Grant 9/30/2026 This grant award provides $1,960,784 in federal funding to the State of Utah for Fiscal Year 2026. Authorized under Section 71119(e) of the Working Families Tax Cut (WFTC) Legislation of 2025, it is the first of two planned awards intended to support the state's implementation of new Medicaid requirements. Small Rural Hospital Improvement Program 2/6/2026 The purpose of SHIP is to assist small rural hospitals with their quality improvement (QI) efforts and with their adaptation to changing payment systems through investments in hardware, software, and related trainings.
