- The State of Utah DHHS and Local Public Health Governance Committee Meeting Approve Minutes: November 17th, 2025 Grant Title Executive Committee Review Description Rules Review/Discussion: Rule # Rule Name Topic R380-50 LHD Funding Formula Language modifications to the rule continued. Other Agenda Items: Other Agenda Topic Discussion Includes Notes Disease Plans: (As an informational item for Governance, the following disease plans were approved in Epi/Informatics Exec at our meeting on Wed Nov 12) Plan Topic Blood Lead Approved in Epi/Informatics Exec Ebola Virus Approved in Epi/Informatics Exec Influenza Approved in Epi/Informatics Exec Hepatitis A Outbreak This plan contains current information on standards, guidance, and best practices to be used by public health departments as they respond to hepatitis A outbreaks. Circumstances may vary based on LHD capacity. Exempt Grants (These grants have been labeled as exempt during the past two weeks): Grant Name Application Due Description State Primary Care Offices (PCO) Grant 12/19/2025 Anna West presented the State Primary Care Offices (PCO) grant. Ashley Moretz made a motion that this grant be designated as exempt, Dorothy made the second, and this passed unanimously as well. This grant will remain exempt through the remainder of the grant cycle and will be reviewed again with the next competitive application (unless there is a change in funding amount or scope). State Loan Repayment Programs 1/12/2026 Recipients can choose the eligible primary care disciplines their programs support and HPSA locations based on need. Mini-Grants Available to Offer Leadership Learning for Professionals Working in Public 10/31/2025 Mini-grant requests can range from $10,000 to $20,000 and are intended to fund the development and offer of learning opportunities. The mini grants are not intended to be used as scholarships for staff attending other leadership training courses or conferences. Next Governance Meeting: December 15th, 2025
