MIDVALE CITY REQUEST FOR PROPOSAL TO SUPPLY STREET LIGHT FIXTURES Midvale City invites qualified manufacturers and suppliers with Rocky Mountain Power qualified product (LDL, DLC) to submit proposals for new high efficient street light fixtures and luminaries. It is the intent of the City to replace all of the streetlight fixtures and retrofit decorative lighting with high efficient luminaries. The primary objectives of the street lighting project are to take advantage of high efficient lighting and to light dark areas for the citizens of Midvale City. The objective of this RFP is to qualify and purchase quality street lights for the project. A detailed Request for Proposal for these items may be obtained from Phillip Hill by email at phill@midvale.com The responses will be evaluated after the closing date and a supplier or suppliers will be chosen to enter into an agreement with the City. The City intends to select a supplier that can provide specified product for a competitive price. All responses (5 copies) must be submitted no later than 3:00 p.m. MDT on Monday, May 20, 2013 to the City Recorder at Midvale City Hall, 655 West Center Street, Midvale Utah, 84047. The City reserves the right to reject any and all responses.

In accordance with the Americans with Disabilities Act, Midvale City will make reasonable accommodations for participation in the meeting. Request assistance by contacting the City Recorder at 801-567-7207, providing at least three working days advance notice of the meeting.

Notice of Electronic or Telephone Participation

Council Members may participate in the meeting via telephonic communication. If a Council Member does participate via telephonic communication, the Councilmember will be on speakerphone. The speakerphone will be amplified so that the other Council Members and all other persons present in the Council Chambers will be able to hear all discussions.
