REQUEST FOR QUALIFICATIONS
SEALED Statement of Qualifications must be received by Wednesday, April 3, 2013 by 2:00p.m. in the office of Midvale City located at 655 West Center Street, Midvale, Utah 84047 for the following project:
GENERAL CONTRACTOR FOR THE
CONSTRUCTION OF THE
MIDVALE CITY MUNICIPAL CENTER
PROJECT DESCRIPTION:
Midvale City is seeking Statement of Qualifications from qualified General Contractors for the construction of a new Municipal Government Center. The City intends to select 4-5 General Contractors from the submitted SOQs who will then be asked to submit competitive sealed price proposals.
INSTRUCTIONS AND QUALIFICATIONS:
General Contractors responding to this RFQ shall have completed at least one (1) similar combined municipal court, and city hall facility.
OBTAINING DOCUMENTS:
Request for Qualifications instructions and forms may be obtained at JRCA Architects Office, 577 South 200 East, Salt Lake City, Utah 84111, and at the Midvale City Offices, 655 West Center Street, Midvale, Utah 84047.

Bidders are instructed not to contact other City staff or elected officials regarding this project except as noted above.
Midvale City reserves the right to reject any or all proposals, or any parts of any proposal, to waive any informality or technicality, and to accept any proposals deemed to be in the best interest of the City.

Notice of Electronic or Telephone Participation
Council Members may participate in the meeting via telephonic communication. If a Council Member does participate via telephonic communication, the Councilmember will be on speakerphone. The speakerphone will be amplified so that the other Council Members and all other persons present in the Council Chambers will be able to hear all discussions.
