How to Cancel Your Registration

We hope you are happy with Utah.gov Registered User Services. If you do need to cancel your registration for any reason, follow these simple instructions.

  1. Please complete the Cancellation Request Form (Adobe PDF Document)

  2. Please fax or mail your completed Cancellation Request Form to:

    NOTE: Please include account number(s) and the effective date of cancellation.

  3. You may receive a courtesy call from a representative to help us improve our service.

Cancellation policy:

New Registrants: If you cancel within 30 days of your initial service start date, there will be a non-refundable fee of $15 (of the $95 annual startup fee). After 30 days, you will be responsible for the entire $95 fee.

Existing Registrants: If you decide to cancel your registration, you will have 30 days from the annual renewal date to do so, or you will be responsible for the $95 fee.

Usage fees are non-refundable.
