- Rocky Fritts request to terminate therapy.
- Jeff Swyers request for termination of probation.
- Betty Long request for termination of suspension.
- Karen Christiansen request for general supervision instead of direct supervision.
- Amy Stone, request for termination of urine screens – additional documentation from therapist regarding focus of treatment.
- Lori Niven, request to be allowed to work in home health.
