Review probation modification requests received before 10/08/09:
- Kimberli Johnson, request to add a second prescribing practitioner. 
-Christopher Singer, request for termination of probation.  
-Kim Baker request for access to controlled substances
-Angie Mangelson, request to terminate attendance at 12-step meetings.
-Sandy Piatt, request to discontinue attendance at aftercare and to increase attendance at AA meetings.  
-Amy Stone, request to discontinue urine screening; request to terminate counseling; request to terminate the requirement to submit personal (self-assessment) and employer reports.
