Review probation modification requests received before 06/11/09:
- Bambi Koeller, requesting to be allowed to work in an agency or home health.
- Keith Moslak, requesting termination of probation. If not granted, requesting termination of attending 12-step meetings.
- Gaylynn Pope, requesting to be allowed to contact any pharmacy or drug supplier for the purpose of filling a prescription order.
- Janet Wells, requesting termination of probation.
- Kaylyn Thompson, requesting access to controlled substances be returned.
- Michael Gearheart, request to modify restrictions.
